The AFFIXUS® Hip Fracture Nail System treats a wide range of proximal femoral fracture indications using a single set of user-friendly instruments, and combines the principles of a compression hip screw with the biomechanical advantages of an intramedullary nail. Listed below are just a few of the main features that make this system unique:
The anatomic bends of the nail are important features. This hip fracture nail includes a 3° distal bend, paired with a chamfer on the front of the distal tip of the nail. Combined, these features facilitate ease of insertion through the proximal intertrochanteric/subtrochanteric region, and decrease the risk of anterior cortex penetration. Also, the 1.8m radius of curvature accommodates the anterior bow of the femur.
GOAL POST™ Technology is designed to facilitate visualization of the femoral neck on the lateral view in order to more accurately place the guide pin for the lag screw placement. The anterior and posterior metal posts on the proximal aspect on the insertion jig allow for an unobstructed fluoroscopic view down to the base of the femoral neck, and assist with fine tuning of the guide pin before it is fully seated in the femoral head.
The AFFIXUS® Hip Fracture Nail comes with a pre-assembled set screw to eliminate fiddle factor and save time in the OR. This pre-assembled set screw engages with the lag screw to create a non-rotational or non-compression device. This set screw can engage the lag screw before or after inserting an optional anti-rotational screw. If compression is needed, one quarter back will allow for dynamic compression.

Comprehensive Sizes
The hip fracture nails come in a range of anatomical options in varying angles, diameters, and lengths:
125°/130° Neck Angles
Short Nails: 9, 11, 13 mm distal diameters
180 mm length
Long Nails: 9, 11, 13, 15* mm distal diameters
260-460 mm lengths (20 mm increments)
*Only 130°, 320-440 mm lengths
Anti-Rotation Hole
The AFFIXUS Hip Fracture Nail implant was designed to include an anti-rotation (AR) hole, which can be used as a utility hole to provisionally stabilize a fractured femoral neck and head segment during the lag screw placement or for placement of an AR screw, if desired.
Lag Screw Design
The lag screw was designed with a reverse buttress thread pattern, which is important for providing subchondral support under load, while also helping minimize potential for “cut-out”. The lag screw has wide slots or grooves providing the surgeon approximately 15° of adjustment when tightening the set screw. Also, the lag screw is unique in that it has a shoulder which prevents medial migration.
User-Friendly Instrumentation
The instruments are laid out in a procedural order and are easily identified. These instrumentations are color-coded to help identify what instruments are needed for each implant component.
Jig Assembly
Since the system offers two different AFFIXUS nail angles, 125° and 130°: the system includes 2, easy to use jig assemblies that correspond to the neck angle of the implant. The jig assembly was also designed to include a jig tab. This jig tab mates with the implant nail slot to ensure both the targeting jig and implant neck shaft angle match.
During use, if it is difficult to attach the nail to the jig assembly, it can simply be confirmed that the nail and jig assembly are identified with the same neck shaft angle.
On the proximal portion of the jig assembly, there are two holes, where a 3.2 mm guide pin can be placed. Under lateral fluoroscopy, this guide pin gives a visual indication to the potential starting point of the 3.2 mm guide pin.
The jig assembly also includes 3 holes that allow a 3.2mm guide pin to be placed through the assembly to help visualize the following:
Top hole:Allows the surgeons to visualize the trajectory of the lag screw guide pin
Middle hole:Shows the top of the nail at the jig/nail junction
Bottom hole:Shows placement of the AR screw on the medial side
Lag Screw Drill
The lag screw drill includes a pre-assembled depth stop device. After taking measurement
of the lag screw, set the measurement length on the lag screw drill by pushing the side buttons and sliding the depth stop forward or backward to the desired measurement length. There is also a notch on the lag screw drill that is visible under fluoroscopy. This notch is a visual reference of 100 mm lag screw length.
Flexible Instruments
The system includes two flexible instruments where flexibility makes sense. When preparing the femoral canal, a flexible entry reamer has been designed to help with the entry on larger patients. This reamer will bend to help gain proper entry into the femoral canal. Also, flexibility has been added to the jig bolt driver. This allows the surgeon to easily tighten down the jig bolt inside the jig assembly.
Compression Wheel
This system includes a one piece compression wheel assembly that can be easily placed unto the lag screw driver by sliding the wheel device unto the lag screw driver without disassembling instrumentation. Rotate the compression wheel clockwise to compress the intertrochanteric component of the fracture.
Reduction Tools
The AFFIXUS Hip Fracture Nail System comes with several useful reduction tools to aid in gaining proper alignment, length, and rotation. These tools are a ball spike pusher, bone hook, femoral bone clamp, pistol guidewire gripper, guidewire pusher and long reduction tool.
